Permission Slip for all summer camps 2024 : My child has permission to go off KPUMC campus for summer camp experiences. As a parent, in consideration of Kessler Park UMC agreeing to take my child on said church-sponsored field trip, I hereby give my approval to his/her participation there. I assume all risks and hazards incidental to such participation including transportation to and from the activities, and I do hereby waive, release, absolve, indemnify and agree to hold harmless Kessler Park UMC, the staff, organizers, sponsors, supervisors, participants, and persons transporting my child to and from such field trip, for any claim arising out of an injury to my child, whether said injury occurs at such field trip or while being transported to or from the same.
